Environmental factors were scored as follow: BMI (≤ 24 kg/m² = 1, > 24 kg/m² = 2); Wrist circumference (≤ 80 cm = 1, > 80 cm = 2); Systolic blood pressure (< 140 mmHg = 1, ≥ 140 mmHg = 2); Diastolic blood pressure (< 90 mmHg = 1, ≥ 90 mmHg = 2); Blood glucose (≤ 6.1 mmol/L = 1, > 6.1 mmol/L = 2); Alcohol consumption (nondrinker = 0, < 25 g/d = 1, ≥ 25 g/d = 2); Cigarette smoking (nonsmoker = 0, < 20 cigarettes/d = 1, ≥ 20 cigarettes/d = 2).
